Getting started is easy! Anyone can refer a patient; patients themselves, caregivers, or medical providers. Fill out the form below and a member of our team will contact you for next steps. You can also call (720) 295-1844 for enrollment assistance.

Patient Referral

We are accepting GUIDE referrals for those with a dementia diagnosis and Original Medicare*

*If an individual has a Medicare Advantage Plan (Medicare managed by a private health insurance company like United, Kaiser, BCBS, etc) they are NOT eligible for the GUIDE program.

If you are not GUIDE eligible, we do offer private pay services. Please contact our office for more information.
